ARCLYFT™ DME/O&P MARKET INTELLIGENCE · DOMAIN 02
Manufacturers, Products & Formulary
Which product differences matter for the people and services you are responsible for—and what should your product choices and formulary account for?
Make product comparisons that account for the device, the service delivered with it and the evidence relevant to the decision.
Why this matters
Product decisions in DME/O&P are often made at the wrong level of detail. A formulary lists a brand or a billing code; an authorization request names a model; a contract prices a code. None of those says whether two products billed under the same code can serve the same members, whether a higher-priced model has a standard coding pathway at all, who fits and supports the device, or which differences actually matter for a member's use. Substitutions can look equivalent on paper when they are not, and arrangements can price the product while leaving the service unstated.
The question this domain answers
What is actually being compared—and which of the differences between those products change the service, the coding, the coverage or the cost of use?
What we examine
See the evidence we examine
- Manufacturer portfolios — which product families a manufacturer makes, where it is broad and where it specializes, and parent/brand relationships that make two "alternatives" the same company.
- Within-catalog distinctions — how products in one catalog are positioned for different users, feature sets and coding pathways, and where those positions overlap.
- Matched cross-manufacturer comparisons — like for like: same product class, same intended use, same service level; specification, manufacturer claim and independent evidence kept separate.
- Product-to-code evidence — the codes listed for a product on the PDAC Product Classification List where Medicare requires coding verification, the manufacturer's own coding guidance where it does not, and the custom-fitted / off-the-shelf pair where it applies.
- Distribution, compatibility, fitting and support — direct or distributor channels, consumables and compatible components, who sets up, trains and repairs, and what the manufacturer commits versus what a vendor must provide.
- Costs associated with use — published allowances as references, plus the service, consumable, repair and replacement elements a headline price omits. Negotiated rates and acquisition costs stay within client-specific work.
A worked product decision: three knee braces under one set of criteria
Illustrative example. Organizations and scenario facts are hypothetical. Not a client result.
The use case. A hypothetical plan is revising its authorization criteria for prefabricated knee orthoses for knee osteoarthritis. Three products are under consideration under those criteria, and the UM team asks whether they can be treated as one item for criteria and substitution purposes.
Educational comparison based on the cited sources and review date. Not a client result or a patient-specific recommendation. Product constructions are as described by the HCPCS code each product is listed under on the PDAC Product Classification List (searched 3 November 2026). Coverage and payment depend on the applicable policy and contract.
Compare the evidence and implications
| Comparison dimension | Össur Unloader One X | Enovis (DonJoy) OA Adjuster 3 | Breg Fusion OA Plus |
|---|---|---|---|
| Construction (from the listed HCPCS code) | Single-upright knee orthosis | Double-upright knee orthosis | Double-upright knee orthosis |
| Codes listed on the PDAC Product Classification List | L1843 (custom-fitted) and L1851 (off-the-shelf), listed since 08/12/2022 | L1845 (custom-fitted) or L1852 (off-the-shelf), listed since 01/01/2017 | L1845 (custom-fitted) or L1852 (off-the-shelf), listed since 01/01/2017 |
| Manufacturer claims | Not compared on this page | Not compared on this page | Not compared on this page |
| Independent comparative evidence | None identified in the sources reviewed comparing these three products in matched populations | Same | Same |
| ARCLYFT interpretation | A different billed item from the other two. Criteria written around the double-upright codes alone would exclude this design by construction, not by clinical judgment. | With Fusion OA Plus: these products warrant a focused comparison under the same proposed criteria. Whether they can serve as policy alternatives depends on their documented indications, limitations, fitting requirements and the plan's clinical review. | As left. |
What the difference changes—ARCLYFT interpretation. For criteria: assess whether the proposed policy accounts for product class and service level—single- and double-upright designs; custom-fitted and off-the-shelf—rather than by brand or a single code family, so that a design is not excluded by accident. For substitution: the single-upright brace cannot be swapped for either double-upright brace on code alone; the two double-upright braces share a code family and are the pair that warrants a focused comparison, not an automatic swap. For the furnishing and fitting service: the service actually delivered—more than minimal self-adjustment by a qualified practitioner, or not—decides whether the custom-fitted or off-the-shelf code applies, whatever the brand. A shared code does not establish interchangeability; a different code does not establish medical necessity for the more expensive device; a different feature does not establish superiority. A PDAC code assignment is a coding decision, not an endorsement and not a guarantee of payment. The organization decides its criteria and substitution policy; these distinctions are inputs to that decision.
What remains client-specific. Which design a given member needs is a clinical determination made by the treating professionals under the plan's policy. ARCLYFT™ Select supports the organization's policy, product, network and reimbursement analysis—criteria wording, comparison of documented indications and fitting requirements, which products its network actually furnishes at which service level, and what its contracts pay. Appropriately authorized clinical decisions remain with the client and treating professionals.
What this changes for an organization
- Criteria written at the right level — product class and service level, not brand names.
- A documented basis for substitutions and upgrades — what was compared, on what evidence, and what remains a clinical judgment.
- Product arrangements that include the service — fitting, training, consumables, repairs and who provides them.
- Fewer inconsistent requests for information, because the questions that matter—model, version, service level, fitting record—are asked up front.
What the evidence establishes, and what it does not. A product specification establishes what the manufacturer says the product is; a code listing establishes how the product may be billed; neither establishes medical necessity, comparative outcomes or clinical suitability for a member. FDA clearance is not comparative superiority, and catalog breadth is not clinical quality. Applying these distinctions to your population, policies, contracts and alternatives is client-specific work.
