O&P Clinical & Operating Expertise: ARCLYFT Health's O&P advisory capabilities are informed by more than 20 years of direct patient care and operational experience across a broad range of orthotic and prosthetic specialties. Spencer Doty is an ABC Board-Certified Prosthetist/Orthotist in good standing and has extensive clinical experience in both adult and pediatric O&P. He also previously served as a paneled provider for California Children's Services (CCS), providing additional experience with the clinical, operational, and coverage considerations unique to pediatric O&P. His former O&P organization also maintained specialized programs in several service areas that have become increasingly niche within the industry, including diabetic and therapeutic footwear, compression garment therapy, burn and scar management, and post-mastectomy care.
DME/O&P Care Delivery Models
How DME/O&P reaches the patient can be just as important as the product being provided. Delivery models vary based on product complexity, clinical need, mobility, urgency, fitting requirements, and the level of education or support required.

The patient travels to a vendor-owned or leased facility, typically for a scheduled appointment. This model generally provides the vendor with the greatest access to inventory, fitting equipment, clinical resources, and additional personnel, allowing products to be evaluated, adjusted, exchanged, or fitted in a controlled patient-care environment.
Facility-based care is particularly valuable when the product requires clinical assessment, fitting, customization, education, or access to equipment and inventory that cannot practically be brought into the field.

Vendor personnel travel to the patient's residence to provide equipment, fitting, setup, education, or other services. Home delivery can significantly improve convenience and access, particularly for patients with limited mobility or transportation challenges.
The tradeoff is a more limited service environment. Field personnel can carry only a finite amount of inventory, equipment, and supplies and typically operate within a schedule of multiple patient appointments. Effective home-based programs therefore depend on appropriate product selection, preparation, scheduling, and processes for resolving issues that cannot be addressed during the initial visit.

DME/O&P may be delivered directly to a hospital, skilled nursing facility, rehabilitation facility, or other care setting when equipment is needed before the patient can transition to the next site of care.
Timely coordination is particularly important when DME/O&P is part of the discharge plan. Equipment availability, authorization, delivery, fitting, and patient or caregiver education can directly affect whether the patient is ready to safely leave the facility as scheduled.

Certain DME/O&P products and supplies may be shipped directly to the patient's home without an in-person vendor encounter. This model can provide a convenient and efficient way to deliver appropriate products, particularly recurring supplies and items that do not require complex fitting or setup.
Its effectiveness, however, depends on more than successful shipment. The correct product must arrive at the correct location, within the expected timeframe, and the patient must be able to appropriately use the item once it arrives.
As direct-to-patient fulfillment becomes more common, vendors should clearly define how their programs address delivery failures, incorrect or damaged products, fitting and sizing issues, patient education, exchanges and returns, troubleshooting, and situations requiring in-person support. These service expectations are important considerations when evaluating vendor capabilities and performance.
CMS-Approved DMEPOS Accrediting Organizations
DMEPOS suppliers generally must be accredited by a CMS-approved independent accrediting organization to enroll in and bill Medicare. Accreditation is intended to confirm that suppliers meet applicable CMS DMEPOS Quality Standards and Medicare program requirements.
