DME/O&P Decision Brief · Industry Practices & Applied Expertise
A Provider Listing Does Not Explain the Service Model
A directory entry tells a health plan that a DME/O&P organization exists at an address. It does not tell you how a patient is referred, who fits the device, how equipment reaches the home, or who responds when something goes wrong.
Three Things a Listing Does Not Establish
A listed location is not a documented service. A registry or directory address shows where an organization is enrolled or listed. Whether a specific service—prosthetic fitting, compression measurement, post-mastectomy fitting—is offered at that location is a separate question.
A documented service is not confirmed coverage or capacity. Evidence that a location provides a service does not establish appointment availability, service area, turnaround time or the ability to take on new patients. Those need direct confirmation.
Missing information is not absence. A service that does not appear in the sources reviewed may simply be undocumented. It is a question to resolve, not proof that no one provides it.
Dispensing Is Not the Same as Fitting
Some items need only to be delivered and explained. Others require measurement, modification, alignment, training or follow-up by a qualified clinician—an orthotist fitting a custom-fitted brace, a prosthetist aligning a limb, a fitter measuring compression or post-mastectomy garments. Treating these as the same service can hide real differences in what a patient receives.
How DME/O&P Reaches the Patient
Vendor facility. The patient visits a vendor location. Well suited to assessment, fitting, customization and education that need inventory and equipment on hand. Watch for transportation and appointment availability.
Patient home. Vendor staff travel to the patient. Helpful when travel is difficult and the service can be performed safely at home. Watch for finite field inventory and time, and for a clear path when an issue cannot be resolved on the first visit.
Hospital or other care facility. Equipment is delivered before the patient moves to the next site of care. Important when fitting, setup or education must be in place for a safe discharge. Authorization, product availability, delivery timing and fitting need coordination so DME/O&P does not delay discharge.
Direct-to-patient shipment. Products are shipped without an in-person encounter. Efficient for recurring supplies and items that need no complex fitting. Successful shipment is not the same as successful service: sizing problems, returns, education and troubleshooting still need a defined path.
No single model is universally preferable. Networks often use several, depending on category, patient need, urgency and geography.
Questions a Plan Needs Answered
How are referrals routed, and who decides which vendor receives an order?
Which items in this category require clinical fitting rather than delivery alone, and who performs it?
What are the delivery, follow-up, repair and replacement responsibilities, and how are they documented?
How are escalations handled when a patient cannot be served as expected?
Which handoffs at hospital discharge need clearer ownership?
How ARCLYFT Helps
ARCLYFT reviews referral and documentation pathways, service-delivery models, vendor onboarding and service expectations, and escalation and follow-up responsibilities. Typical outputs include a workflow review, a responsibility map or an implementation-priority brief. ARCLYFT does not furnish equipment, route referrals, authorize care or take over clinical or payer functions.
